Cook the spaghetti: Bring a pot of salted water to a boil and cook spaghetti until al dente. Drain and set aside.
Sauté the base: In a large skillet, cook ground beef over medium heat until browned. Add onions and cook until soft. Stir in garlic and cook for 1 minute.
Make the sauce: Add crushed tomatoes, tomato paste, sugar, basil, oregano, salt, and pepper. Let it simmer on low for 15–20 minutes, stirring occasionally.
Add glamour: Stir in heavy cream (if using) and Parmesan cheese. Let it melt and thicken the sauce for a few more minutes.
Combine: Toss the cooked pasta in the sauce until well coated.
Plate it beautifully, garnish with parsley or basil, maybe a little extra cheese.
